*This form must be submitted within two weeks of your event. If you are unable to meet this deadline, please contact Greek Life.

You must upload verification in addition to filling out this form or it will not be accepted. For monetary donations, attach a copy of a check or a letter of confirmation from the agency receiving the donation. For non-monetary donations, attach a copy of a receipt of the items donated.
For any documentation of Dance Marathon fundraising, only complete the form for your organization once you have a final total. You may only submit DM funds raised the semester DM occurred.
